2. Fill in all of the name, address, and payment information at the bottom of the form. This is not an online registration form. You must print out the form and either fax or mail it in. When you type information into the form and print it out, none of that information is retained by the website (for your security and protection).

All credit card, social security or other sensitive personal information on paper is shredded after receipt and not sold or shared with outside agencies or individuals. The one exception is that we must give information (name, address, SS#) on credit students to the University of Wisconsin–Milwaukee.
